Primary Objectives of Position

  • Ensure the smooth running of activities, programs and events of the Community Home.
  • Plan and execute Home activities and programs to fulfill the funders’ KPIs. 
  • Work closely with donors, volunteers and stakeholders to provide social and recreational activities within the Home and in community.
  • Support the Social Work Department and Nursing team in liaising with the seniors.
  • Submit monthly events, programs reports to Centre Manager.
  • Work closely with Senior Program Executive to organize activities, build relationship and trust with donors, volunteers and stakeholders.  

Job Responsibilities:

  • Plan and execute both big and small scale activities /programs to keep the seniors engaged and occupied.
  • Plan and execute interest based activities to cater to the needs of interested seniors.
  • Maintain proper documentation of all activities and seniors attendance for EPES submission.
  • Maintain proper documentation for audit purposes.
  • Work with community partners and volunteers for organizing activities for seniors.
  • Participate and contribute in Individual Care Plans discussions.
  • Support the Home in operations and administrative work. 
  • Support in identifying areas of improvement in models of care and service delivery based on evidence-based practices.
  • Respond to crisis situations based on organization’s standard operating procedures.
  • To be clear and adhere to centre’s SOP and service requirements at all times.
  • Ensure clients’ confidentiality is maintained and adhere to PDPA policy when documenting via ICMS or sending our clients details/information to stakeholders.  

Requirements

  • Possess a Diploma or Degree in any discipline
  • Minimum 2 years of relevant working experience in the social service or healthcare sector running programmes/activities for the elderly
  • Passion in working with the elderly and willing to go the extra mile
  • Able to work independently and in a team
  • Excellent communication skills is a plus
