Primary Objectives of Position
  1. Be part of a collaboration to develop and implement a funded pilot project to deliver targeted interventions in neighbourhoods with high healthcare utilization, from operationalization to review and evaluation
  2. Support in the active engagement of residents and community resources to build a resilient neighbourhood
  3. Provide administrative support to the team and collect data & statistics for regular report submission

Job Responsibilities

  1. Support in programme development and implementation through ground-sensing work and gather data through geographical mapping, data analytics, and interviews
  2. Assist in care coordination for the residents and work with community partners to meet the population’s health-social needs and in meeting programme KPIs
  3. Support in providing information & referral to healthcare or social services, conduct follow-up calls and home visits as required
  4. Coordinate and provide logistics support in outreach activities in the community
  5. Support in forming positive working relationships with the residents, and health and social service partners to identify strengths and assist in the formation of ground-up solutions in the neighbourhood
  6. Consolidate data & statistics, track progress of programme and manage database for regular report submission
  7. Carry out relevant administrative duties required by the service
  8. Contribute to department’s projects and committees as and when required
  9. Perform any other duties as assigned

Minimum Education / Qualifications

  • Minimum Nitec in Community Care & Social Services from a recognized tertiary institution or equivalent

Minimum Years of Relevant Experience

  • Preferably with 2 years of relevant experience in community, healthcare or social services

Knowledge/Skills

  • Proficient in MS Office applications
  • Ability to speak second languages or dialects is an advantage
  • Strong team-player
