Job Responsibilities:

  1. Perform health assessment of clients’ condition
  2. Deliver evidence-based care in collaboration with inter-professional teams
  3. Facilitate admission, transfer and discharge of clients
  4. Facilitate transition of care coordination within and between teams and across settings
  5. Monitor compliance of infection control and practices in the clinical area
  6. Administer self-management plans to empower clients, family members and caregivers to facilitate participation in own health management
  7. Implement individualised nursing care plans  for the clients
  8. Participate in medical emergencies or crisis situations
  9. Monitor for early signs and/or deterioration of clients' conditions with appropriate physiological assessments and observations
  10. Implement clinical risk and quality improvement initiatives or activities
  11. Monitor individual standard of practice with available benchmark data
  12. Report near miss and error reporting using organisational incident reporting system
  13. Precept new staff nurses, enrolled nurse, or healthcare assistants in learning activities
  14. Provide feedback on clinical performance of enrolled nurse and healthcare assistants   
  15. Understand MISSION Framework, use relevant tools to support the Professional Practice of self and the team members
  16. Ensure clients’ confidentiality is maintained and adhered to PDPA policy when using systems such as NEHR, and when handling confidential information to stakeholders

 

Other Responsibilities

  1. Coordinate and support the Healthcare Assistants in running table-top activities, events and/or outings in collaboration with Programme Executive / Community Partnership colleagues
  2. Assist supervisor(s) in schedule planning for staff and clients
  3. To perform any other duties as required by the Operations Manager, Nurse Manager, Deputy Director, Director,  CEO and the AWWA Management

 

Knowledge/Skills

  • Knowledge of Mandarin/Malay/Chinese Dialects to communicate with the elderly is an advantage;
  • Good communication and interpersonal skills;
  • Basic IT skills.
