Case Management

  1. To provide support to EIC operations such as centre visit, initial screening, enrolment of new students, class support, internal transfer, team discussions,transition of Graduands, referral to relevant programmes etc.
  2. To conduct needs ( biological, social, psychosocial, financial) and risk assessment for vulnerable families, identifying and clarifying the key issues to the situation through classroom observations, phone conversations, face to face consultations and/or home visits.
  3. To prioritize and plan interventions ( via case work, group work or community organisation) to achieve set goals and objectives. To liaise with helping agencies (FSC, HDB, MOH, MSF, hospitals- KKH, NUH, SCS, Sg Enable etc) to provide resources to the families in need.
  4. To work wth professional colleague on the Individual Caregiver Support (ICSP) and follow through with the family. To work closely with the professional colleagues ( interventionists, therapists, psychologists) to ensure suitable & conducive environment for child’s learning and development, identify problem behaviours and discuss the management of such cases with relevant staff and parents.
  5. To update and maintain all case recordings and documentation


Programme Planning

  1. To create and conduct programmes that are informed by needs assessment of the families at AWWA EIC with an aim to engage and empower the families for eg. workshops, family outings, collaborations with relevant agencies/volunteers, group work with parents, grandparents, fathers or siblings, play groups etc.
  2. To work closely with the transdisciplinary team in the planning, implementation and evaluation of group and community programmes
  3. To maintain and update all programme records and documentation


Training & development

  1. To apply the principles and protocols as guided by SASW professional Code of Ethics
  2. To participate actively in case consultations, group supervision, training programmes and continuing education for personal and professional development
  3. To be engaged in evidence-based practice through  keeping up with research findings on relevant practice areas, and  tracking current trends by keeping up with updates and news of the disability sector

Any other relevant duties as assigned by supervisor or Centre Manager,Director, Group Director or Management
